GOVERNMENT SUBSIDY www.dental.wa.gov.au
Subsidy is available to patients who hold a current Health Care Card or Pensioner Concession Card. You will need to bring your Emergency Authority Form or your General Authority Form to DB Dental when you attend your dental appointment. To obtain these forms you will need to speak with the staff at the Public Dental Clinics or visit their above website for further details.
